Medical Social Worker

Our Medical Social Worker will provide referrals to community resources. They also offer short-term counseling for patients and caregivers, as well as:

• Assist with living arrangements, financial concerns, and long-term care planning
• Provide information and assist with Advance Directives
• Focus on overall social and emotional health
